- Early intervention support packages to young people with low to medium support needs who are homeless or at risk of homelessness.
- Post Crisis Support case management support to young people transitioning to independent living (social, public or private housing) from supported accommodation.
Criteria
- High Needs
- young people 16-25 years who have a history of chronic homelessness, repeat users of crisis accommodation services and rough sleeping
- willingness to engage with an allocated case worker.
- Early Intervention support packages
- Young people 12—18 yrs who have not engaged in supported accommodation and
- at risk of homelessness, disengagement from family, school, education, training/employment.
- Post Crisis Support
- Young people aged 18—25 yrs who are exiting supported accommodation into independent living who have been assessed as demonstrating independent living skills and
- willingness to engage with case management.
