Accommodation and casework support for young people aged 16-18 years who are transitioning from Out-of-Home-Care. The program helps them to develop the skills required for independence.
A range of accommodation options from transitional supported group homes to individual tenancies with Community Housing providers are provided.
The program is available to young people aged 16-18 who:
- are under the parental responsibility of the Minister and
- are unable to continue living with foster carers or return home and
- are leaving OOHC to live independently but are not ready to manage on their own, or
- have already left care, but are not coping with independent living, or
- have been assessed as having the capacity to live independently
Individual programs are available to:
- Young people transitioning from a High Needs Placement (statewide) referred by Community Services Metro ISS team
- Young people transitioning from OOHC in the Sydney Metro West Region referred by Community Services Metro West Regional Child & Family Unit.
